Condition management is a confidential, voluntary program designed to help people with specific conditions stay as healthy as possible for as long as possible. Health management nurses work over the telephone with PPO plan participants who are living with one of the following conditions:
Health management programs extend—but do not replace—the medical care you receive from your doctor. In fact, Blue Shield or Anthem Blue Cross will notify your doctor when you participate in one of these programs. Many doctors are happy to hear that their patients are participating in condition management services offered by their medical plans.
You can decide to participate in condition management programs at any time. The choice is always up to you.
