The health assessment is an online questionnaire (hosted by HealthMedia) about your current health and health habits.
The health assessment will take you about 15-20 minutes and you will get a personalized health report detailing what you can do to improve your health. Your individual results are completely confidential (as federal HIPAA privacy laws require) and are not shared with SISC, your district or anyone else.
