The health assessment is an online questionnaire (hosted by HealthMedia) about your current health, personal and family medical history and health habits.
There are two versions of the assessment. The longer version is very comprehensive and includes a place to record your health numbers (blood pressure, blood glucose, cholesterol, etc.). The shorter "snaphot" version takes only 10 minutes to complete and you do not need to record your health numbers.
Based on your responses, you get a confidential personal plan with suggested actions. Your individual results are completely confidential (as federal HIPAA privacy laws require) and are not shared with SISC, your district or anyone else.
